REACH Day Treatment
Oakville Trafalgar Memorial Hospital

REACH is a day-treatment classroom for adolescents experiencing mental health concerns that require more intensive support than can be provided in standard outpatient care setting/high school classroom setting.

Serves students from grades 6-12.

Stays are expected to be between 6 – 8 weeks. This time will vary based on student need.

Emphasis is placed on academics and Healthy Active Living activities by two qualified teachers and educational assistant.

A social worker, psychiatrist, and child and youth counsellor associated with OTMH provide therapeutic interventions including cognitive behavioral therapy and family therapy.

Referrals are through an assessment completed by the student's health care professional and referred to the classroom through Halton Health Care’s outpatient mental health services department.

Upon admission, family appointments will be arranged. Families are expected to attend programming to assist in their child's treatment.

Following their stay in the classroom, students will return to their home school with recommendations from the REACH teacher about academics.  The therapeutic team will provide information for follow-up care based on individual needs to community agencies, the family physician, a psychiatrist, or a specialized program.
